Food Storage, Sanitation & Safety
One file was missing evidence of 2 doses of MMR, the application, references, interviews, current first aid and CPR, evidence of BRC Suitability, EEC Essentials training certificate. One staff file was missing evidence of Teacher Certification.
Shelves in all rooms were not secured down. The TV on the shelf was not anchored to the shelf. The home living unit was not secured.
The children were observed using playground equipment that required fall zones. The ground was frozen covered in snow and ice. The Boat did not have an adequate fall zone of six feet all around the structure, it was about 3 feet from the fence.

The toy shelves in all rooms were not secured down. The tops and the sides of the shelves were covered with cloth and not washable. Some wooden shelves were worn down and not washable.

The educator files did not have evidence of BRC suitability.
Indoor Space
The outdoor space had a climbing structure that was cracked on the windowsill and missing the slide from the platform to the ground, leaving an opening children could jump out of or fall from. The Director reported the children are using the climbing structures except the dome, the ground is still frozen.

The educator reported evacuation drills are not done monthly to their knowledge.

Administration
The Bleach/Water Solution was stored within the children's reach.
The program capacity is 20. There is not a full time administrator on site.
There were 14 children between two adjoining rooms with one educator, who was positioned in a place she could not see all children at all times.
Supervision of Children
The Staff/Educator Schedule reflected 1. There is not a Teacher qualified person covering Teacher breaks. 2. Only 1 certified Teacher is scheduled when 2 are required.
Equipment and supplies: free of entrapment hazards
There is no fence alongside of the parking lot. The parking lot leads to a busy main road.
The fence outside on the playground had two head entrapments. The tops of the wooden fence were broken and splintered.
A review of 2 staff files was completed, 2 staff have a expired BRC.
