Supervision of Children
While observing the infant classroom, two educators were on one side of the classroom, while a infant was out of sight in their crib in the sleeping room. The sleeping rooms had loud noise machines that made it hard to hear children and the educators in the classroom.

The infant fridge had 2 full bottles and one sippy cup with no label.
A child in the preschool room with Benadryl for an allergy to peanut, egg, milk and shellfish did not have a prescription or medication consent form signed by the doctor for the Benadryl.

The toddler classroom had stacked chairs on the table tops. A toddler was playing with the chairs on the table.
The table was not washed and sanitized prior to children sitting for snack in the toddler room.
The Albuterol in the preschool classroom was missing the original container and prescription label.

The toddler classroom attendance had 5 children signed in, there were 8 children in attendance.
While observing the toddler classroom there were 8 children and 2 educators. During the transition from free play to snack, all 8 children were brought to the bathroom by 1 educator who told them to sit on the floor wait their turn to wash their hands. While the other educator moved the tables and chairs and put out snack.
The thermometer in the toddler room refrigerator was reading 38 degree's. The thermometer in the infant rooms fridge was reading 50 degrees.
The bottom of the bathroom door in the toddler classroom had peeling paint.
Infant are sleeping in dark spaces, it is hard to see their faces. A infant was laying on the floor drinking his bottle. Infants were is saucers and swings awake but in dark areas of the room.
Child Guidance & Interactions
While in the office reviewing documents, I could hear a teacher telling a child no thank you, go to sleep, enough, time to sleep. The infant was crying for about 5 minutes. The teacher was not heard calming the child or nurturing them. She gave two word commands repeatedly.

The wooden house on the preschool playground was chipping paint and had pieces of wood splintering. A rusty screw was sticking out of the back right side of the wooden house.
Missing evacuation procedure in between toddler and infant room. Missing map and procedure in toddler to the hallway.
The emergency medication in the preschool classroom did not have a prescription label.
Individual Health Care Plan (IHCP) - R Apr 2025
A IHCP plan was in the child’s file, but expired. The educators were aware of the children’s special conditions.
