Licensor randomly reviewed 5 of the children's records, and observed the following: ** 1 ** - school age child but does not have the school physical section filled in, out of date signatures in the enrollment packet ** 2 ** - out of date signatures and physical/immunizations ** 3 ** - out of date signatures and physical/immunizations ** 4 ** - out of date signatures and physical/immunizations ** 5 ** - out of date signatures and physical/immunizations.
Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor space (fenced in back yard). The following hazards were observed: ** KITCHEN **- drawer across from the door has multiple hazards inside and the child proof lock is not catching all the time, cabinets under the sink have cleaners inside and they weren’t locked during the visit ** DININGROOM **- left drawer in the hutch has small chokable items inside, multiple outlets not covered ** PLAYROOM **- no hazards ** LIVINGROOM **- drawer with matches and a lighter inside ** BATHROOM **- laundry detergent, borax, and laundry basket full of the same all on top of the dryer. They need to be made inaccessible to the children. ** OUTSIDE **- no hazards.

Progress Reports
Licensor randomly reviewed 5 of the children's progress reports. Of these, 4 were complete and up to date, but the 5th report was out of date.
All of the required items were observed in the1st aid kit. However, the children's emergency contact forms were out of date.
There are 2 exits from the licensed space, 2 separate front doors. Both exits were clearly marked. However, 1 door was not accessible as it had several items blocking it.

Progress Reports
2 of the children had complete and up to date progress reports. Educator stated all of the other progress reports are overdue and she has been working on them.
All of the required items were observed in the first aid kit. However, the instant cold pack needs to be replaced and the children's emergency contact forms need to be updated.
Licensor inspected all of the indoor, licensed space during the monitoring visit. The following hazards were observed: **KITCHEN** - drawer with batteries and other hazards has a child proof tab that is not working, cabinet under the sink has cleaners inside and the child proof lock was not engaged during the visit **DININGROOM** - no hazards **PLAYROOM** - no hazards **LIVINGROOM** - decorative tree is a tipping hazard **BATHROOM** - no hazards **OUTSIDE** - fenced in back yard, no hazards

Licensor inspected all of the licensed, indoor space during the renewal visit. The following hazards were observed: **KITCHEN - no hazards **DININGROOM - no hazards **PLAYROOM - no hazards **LIVINGROOM - tv needs to be anchored as it is a tipping hazard **BATHROOM - laundry detergent on the floor was accessible.
Educator tracks the children's attendance in a record keeping calendar. The time of arrival and departure are noted daily for each child. However, at the time of the visit on 6/27/23, no attendance had been taken on Monday. 6/26 or Tuesday 6/27.

Educator completed a fire drill every month from January through April. However, a drill has not been conducted since 4/19/23.
This investigation inspection recorded no violations or advisories.

Educator has a new household member and had not reported this to EEC. This person has lived in the home for approximately 9 months already.

Progress Reports
Licensor reviewed 5 of the children's records. Of these, 4 were complete and up to date; the other1 was missing a progress report.
No immediate health risks were observed during the monitoring visit. Tested the combination smoke and carbon monoxide detector in the dining room and it worked appropriately. All of the required items were observed in the first aid kit. However, the children emergency contact forms are out of date.
All of the required items were observed in the first aid kit, except for a roll of gauze.
Licensor reviewed one of the children's records. There was no lead testing. Please review all of the children's records and ensure they are complete and up to date.

During the virtual visit, Licensor was able to inspect 5 of the children's records. Licensor observed the following: ** 1 - out of date progress report ** 2 - out of date progress report and signatures ** 3 - out of date signatures ** 4 - out of date progress report, no signature for basic transport in enrollment packet, no physical or immunizations ** 5 - out of date physical and immunizations, out of date signatures. Please inspect all of the children's records and ensure they are all complete and up to date.
Educator has not completed a fire drill since June and she has been open during the past months. Educator does not typically alternate exits, discussed that this should the exit should alternate every month.

Indoor Space - free of hazards
There are 2 separate front doors. Both exits were easily accessible. One door was clearly marked. The other door did not have an exit sign. Educator recently replaced this door and had not put the exit sign back up yet.
Persons on Premises
Licensor inspected all of the indoor, licensed space during the monitoring visit, as well as the outdoor playspace. The following hazards were observed: **KITCHEN/DININGROOM - no hazards in kitchen - power strip hanging from back of chair and outlets not covered in it ** PLAYROOM - no hazards ** LIVINGROOM - no hazards ** BATHROOM - no hazards ** OUTSIDE - fire pit doesn’t have a cover, pile of broken fencing (wood pieces) stacked on top of each other needs to be made inaccessible to the children.
The emergency contact forms in the first aid kit need to be updated. All other required items were observed in the first aid kit.
During the virtual visit, Licensor was able to inspect all of the licensed space and observed the following hazards: ** Kitchen/Diningroom - no hazards ** Livingroom - TV needs to be anchored as it is a tipping hazard ** Playroom - outlets not covered ** Bathroom - no hazards.
