Current consent for administered medications
A preschool teacher was observed repeatedly reminding children to "eat up" before the timer went off. At one point, the educator was overheard saying, "I shouldn't be hearing any voices, only the sound of you chewing."
A review of the evacuation log showed they are not being conducted on a monthly basis.
Through a review of emergency medications it was noted the program did not have the parent's consent to administer an Epi-pen or Benadryl.

An educator was overheard using a harsh tone with a toddler rather than redirecting in a positive way. The child was then forced to sit on the floor.
A small gap between and outlet and the wall was observed in the infant/toddler classroom.
A review of two IHCPs showed one was expired.
A stepping stool was observed stored on top of the handwashing sink in the infant/toddler classroom causing a hazard as it could easily fall off and hurt a child.

Equipment is clean, safe, secured, and poses no hazard.

Accurate attendance; arrival/departure times
A review of the staff record checklist and two staff files showed 5 staff members had expired physicals, and one was missing evidence of[REDACTED]
An educator was observed on the playground with 11 preschool children.
A review of the infant attendance showed eight children signed in, however there were seven children present.

Unsecured shelves were observed in the Toddler/Preschool and Infant/ Toddler classrooms.

The infant/toddler bathroom had a mop bucket and mop, air freshener, and cleaning supplies stored within reach of the children. Diaper cream was observed stored in unlocked drawers in the infant and infant/toddler classroom.

Medication Administration Policy
Upon review of a child's IHCP, it was discovered the parent filled out the IHCP and medication authorization form. It was also noted that not all the required information was completed.
Upon review of medications, [REDACTED] was not in its original container nor was there a prescription label attached.
The following furniture was not secured: 1. a book shelf in the preschool room 2. The dramatic play kitchen in the infant/toddler classroom
A review of an IHCP showed Benadryl was required as part of the plan, however there was no [REDACTED] for the child in the program.
