At the time of the visit there was an [REDACTED] that did s not have a prescription label attached nor was it in the box it was dispensed int.
At the time of the visit one child's consent form was expired. The consent form had a start and end date that had expired.
Written emergency evacuation procedures were not posted by each exit in classroom C or E. The diagrams of escape routes were posted.

Children were observed eating morning snack on the rug, there was no barrier between their snack containers and the rug.
2 children were observed going outside into the snow without snowpants, or gloves, the program provided socks for their hands, 1 child did not have a hat. Extra outdoor clothing must be available to ensure children are protected from the weather.

Choking hazards were found in the Toddler E classroom, erasers, pasta, wooden candy corn and elastics.
1 child's [REDACTED] prescription label stated to discard after 1/7/25.
During a review of staff records, two files did not have evidence of documentation of Interviews or verification of 2 References.
In Room B, there wasn't evidence of a method of knowing exactly who is present in the classroom at any given point in the day.
Written plan detailing procedures for emergencies were not posted.
There wasn't a barrier between the children’s space in the hallway and the kitchen. A paper cutter was observed on the kitchen table and accessible to children when transitioning throughout the halls.
