There were three people who are assistants in the program, two full time. The educator does not keep assistant's attendance records.

The educator has an alternative play space agreement and she uses vehicles to transport the children to the playground. Missing written transportation plan, policy and procedure, written parental consent to transport, proof of required insurance and 7D driver's licenses.
The intended program is located in an apartment building on a busy street. There is no appropriate play space area on the property that is completely enclosed. There is also no appropriate, identified alternative play space. Please identify and outline outdoor play space, and include a plan on how children will be safely transported to play area.

There was chipping paint on a back shed and a back wall of the home. Please ensure there is no chipping/peeling paint anywhere on the property; confirm once completed and a follow up visit will be scheduled.
Licensor walked through indoor, licensed space and there were no hazards observed. Licensor also walked through outdoor space (fenced in back yard) and the following hazard was observed: There were parts of the side and front fence that were not stable. Please ensure all fencing around the area are 4 feet high, permanent and sturdy. Confirm once completed and a follow up visit will be scheduled.
