The licensor observed a (REDACTED) to be napping in a pack and play on her back with a sleep sack. The pack and play mattress was not installed properly leaving a gap at the end of the pack and play. This is the Educator's second citation of safe sleep, the first one was in April 2024.

The Educator did not have accurate attendance records. The Educator stated that an (REDACTED) began attending the program one time a week at the end of January but the (REDACTED) was not on the attendance until the beginning of March.

The licensor observed the doors to 2 rooms with napping children were closed. One of the rooms had a (REDACTED) napping. The other 2 children were (REDACTED)
The licensor observed a (REDACTED) to be napping on a mattress that was not the mattress that came with the pack and play. The mattress was too big for the pack and play and did not fit correctly. The mattress was a soft mattress.
