EEC observed an infant feeding themselves a bottle while lying on a bouncy seat. Please ensure infants are held while being fed a bottle.
Some of the electrical outlets were uncovered in two of the classrooms. Please ensure all electrical outlets within the reach of children younger than school age are inaccessible by use of a safety device or covering that prevents access to the receptacle openings.

The bottles in two of the classrooms were not labeled with the name of the child. Please ensure personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
The program did not conduct or document regular staff meetings in February, July and October 2024. Please ensure to document and conduct regular staff meetings each month.
Emergency medications readily available
An unanticipated non-prescription for mild symptoms that was prescribed as part of a child's Individual Health Care Plan was not on site. Please ensure all unanticipated non-prescriptions prescribed in an Individual Health Care Plan are on site at all times.
There was an educator without current First Aid training. Additionally, a new staff did not complete the EEC Essentials 2.0 prior to working in the program. Please ensure all educators obtain current certification of training in basic first aid appropriate to the population served and ensure all applicable staff complete the appropriate training package prior to working in the program.

At the time of the visit the following documentation was missing and/or expired: (1) Individual Health Care plan was expired, (1) individual Health Care Plan was missing the physician signature with a date, and (1) {REDACTED} plan was expired.

In two of the classrooms the radio wire, speaker wire, and computer wires were longer than 6 inches accessible and within reach of the children.
In the {REDACTED} classroom the paint was chipped and peeling from the wall, (1) L brackett was not flush against the wall, and in the {REDACTED} classroom the back of a shelf was not adhered to the shelf.

The program did not following proper hand washing policy as the following was observed: a child coughed in their hands and did not wash their hands, several {REDACTED} children had runny noses and were touching their noses then toys not washing their hands, and a child in the {REDACTED} classroom was in the bathroom and did not wash their hands.

The attendance was inaccurate in one of the classrooms. A child that went to another classroom was not signed out. Please ensure to maintain accurate attendance records.
Two adult's medications were accessible to children in one of the classrooms. Please ensure all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags, and purses are locked or inaccessible to children.
An emergency medication was not immediately available. The medication was stored in another classroom far away from where the child was. Please ensure emergency medications are immediately available for use and accompany the children anytime they leave the facility in the care of staff.
The program did not have record of previous menus for one year. Please ensure to keep previous menus on file for one year.

In the {REDACTED} and {REDACTED} classrooms the attendance did not accurately reflect the number of children currently present. The {REDACTED} classroom had (5) children signed in on the attendance however there were (6) children present. In the {REDACTED} classroom there were (11) children signed in on the attendance however there were (14) children present.
One {REDACTED} child went to the bathroom unsupervised as the child did not use the bathroom. The child put a variety of pieces of toilet paper in the toilet then flushed the toilet.
The {REDACTED} classroom curriculum did not provide a well-balanced curriculum of specific, planned learning experiences that support the social, emotional, physical, intellectual and language development of all children.
The snacks located in the kitchen did not have the expiration date labeled on the bag.
The snacks located in the kitchen are not being stored in covered containers.
The program does not have evidence of conducting and documenting monthly practice fire drills.
