Licensor observed that CHILD1 and CHILD 2 were missing educator’s name on permission forms. The topical medication was not listed.

Licensor observed the following indoor hazards: BEDROOM: 1- the dresser and the tall armoire were not secured to the wall. 2- the radiator was missing the side cover. 3- there were several beauty products accessible on the top of the dresser. 4- there was a loose extension cord accessible.

Random review of children records. - Child 1 was missing physical description, anticipated days of attendance/arrival/departure times, specific places on permission form, educator/assistant’s names on permission to transport to medical facility and received emergency medical treatment, topical medication/sunscreen not listed. The emergency card was missing educators’ names and child’s name. - Child 2 was missing time of attendance and educator’s names on permission form. Specific places was not listed.
