Licensor observed that the educator and assistant did not have current CPR/FA certification. - Educator and assistant CPR/First Aid Exp Date: 03/28/2026. The educator must cease providing care and may not resume until valid, up-to-date CPR and First Aid certifications are obtained, in accordance with EEC health and safety requirements.
Licensor conducted a random review of children’s records and observed the following: Child #1: Missing annual updated form and an updated physical. Child #2: Missing annual updated forms.

During the visit, three progress reports were reviewed. One child’s progress report (Child #1) was not updated.
Random reviewed of children’s records. Licensor observed the following: CHIID 1: was missing physical description, lead test, receipt of parent handbook was missing parent's signature. CHID 2: missing physical description, date and parent's signature on page 3, receipt of parent handbook and visit notice were not signed by parent. CHILD 3: missing permission for walks, the physical was not updated.

Licensor observed that hot water temperature in the bathroom sink was 140 F. Hot water cannot exceed 120F.
Licensor observed that the mirror in the bedroom was not secured to the wall. There were four outlets uncovered. The TV was not anchored. The toy shelf in the playroom was not secured to the wall.
This monitoring visit inspection recorded no violations or advisories.

Random review of children’s records. Licensor observed that Child 1 was missing permission forms and physical exam. Child 2 was missing the physical exam and the annual update form. The permission form was missing assistant’s name. Child 3 was missing the annual update form.
Licensor observed that the program did not have a personnel file on her assistant.
1- Licensor observed a treadmill and clutter accessible in the reading area. 2- Licensor observed that the pool ladder was accessible. The pool was empty.
Licensor observed that the sleeping mats were not stored in a sanitary manner.

Licensor observed that 2 children did not have enrollment forms on file.
Licensor observed that the program was using a converted garage that has not been approved by EEC. Licensor inspected the new room and approved it during this visit.
Licensor observed a {REDACTED} child sleeping on a changing pad with a small flat pillow and bib attached to the pacifier. This is the educator’s first safe sleep violation. The RA was sitting next to the child.
