At the time of the visit a toilet plunger was accessible to children in the preschool/toddler bathroom.
At the time of the visit six children requiring emergency medications were reviewed and the following was found: two children were missing a portion of their medications, and two children had expired "discard by" dates on their prescription labels.

The bathroom used by the toddlers and preschoolers had two areas with exposed rust in need of repair.
This investigation inspection recorded no violations or advisories.
The following was observed to be accessible to children: -In TODDLER 1, the changing table doors were unlocked and glass cleaner, floor cleaner, and loose garbage bags were found. -In INFANT 2, the changing table doors were unlocked and spray cleaners were observed.

Chipping and peeling paint was observed on the outside of the building to the left of the canopy in the small playground space and observed on all walls of the outside of the building where the large playground is located. All areas were accessible to children. Peeling paint and sharp edges on the lower orange wall in the PREK room to the left of the door leading to discovery preschool were accessible to children.

During a walk through the following was observed in TODDLER room (older children): -Three broken floor tiles in front of the exit door accessible to children - Adult Scissors and a hole puncher were observed in an unlocked drawer accessible to children. These items were moved during visit. During a walk through the following was observed in TODDLER room (younger children): -A loose bag was observed in the unlocked lower cabinet accessible to children. The bag was moved during the visit. During a walk through the following was observed in PREK room: -A phone cord longer than six inches was accessible to children. The cord was secured during the visit. - Rust on the bottom of the exit door accessible to children - A broom leaning against the wall next to the phone was accessible to children During a walk through the following was observed in DISCOVERY PRESCHOOL room: -A phone cord longer than six inches was accessible to children. - Ceiling vents covered in dust above the door to the Toddler Room. - A broom leaning against the wall was accessible to children next to the door leading to the toddler room. During a walk through the following was observed in PRESCHOOL room: -A phone cord longer than six inches was accessible to children. The cord was secured during the visit. -A wall cabinet to the left of the see-through window is peeling and in need of repair.

A review of five child records reviewed indicated: - One child record does not have a current physical. The last physical was dated 3.31.2021 -One child record did not have a current medication authorization form signed. The last medication authorization signed was dated 9.2.2021.
Playground Safety - Fallzones
The ground surface area under the swings and slides was inadequate at approximately two inches of mulch.
The following was observed during the walk through outside to be accessible to children: -a hose longer than six inches between the fence and the perimeter of large playground. -four spots of uncovered roots that appear to be from a tree or plant located near the swings is considered a tripping hazard. -Unknown vegetation growing near the swings.
A review of the emergency evacuation logs indicated: Drills were not conducted in July or August 2022.
