The licensor reviewed 5 of the children records. The following were observed: 1.Two of the children records did not have a picture of the children or a physical.description of the children. 2.One of the children record did not list the educator or assistants names on the general permission for basic transportation and to transport to medical facility.. (Educator wrote “all teachers" instead of the names”. 3.One of the children record did not have an annual update. The parent last signature was 2/11/25. 4.One of the children record did not specify where the child can go when off the premises. 5.Two of the children did not have an updated physical.(The physicals were due 3/2025 and 7/2025. 6.One of the children record did not have all information filled out (see page 4 and page 5 of the enrollment, developmentally history “top and bottom” And social relationships. 7.One of the children record did not list the educator or assistants names on the general permission for basic transportation and to to medical facility.

Staff Records
Licensor observed that the educator did not have proof that she demonstrate and successfully complete CPR/FA techniques to a certified instructor. CPR/FA EXP: 07/20/2023.
Licensor observed that the regular assistants did not have copies of their Medical Forms on personnel files.
The educator did not provide appropriate supervision to 10 children that were in the basement with an unapproved caregiver when she opened the door for this Licensor at the kitchen on the first floor.
Adult-Child Ratios
Upon this licensor’s arrival at 2:20pm, the educator was providing care to 10 children by herself, including 8 children under the {REDACTED}. There is a young lady in the program assisting her but she is not an EEC approved caregiver as of today. 2. There are 10 children present at the time of the visit including {REDACTED} under the age of {REDACTED} ({REDACTED}, {REDACTED}, {REDACTED}, {REDACTED}, {REDACTED}, {REDACTED}); {REDACTED} and 3 {REDACTED}.

The educator did not have evidence that current caregivers assisting her are licensed by EEC.
The program is exceeding its licensed capacity at the time of the visit because the educator was providing care to 10 children by herself. The educator did not have the number of qualified staff to meet ratios.

1. There is a young female on the premises identified by the educator as a “volunteer.” There is no BRC on file for this “volunteer.” 2. A young female arrived at 2:40pm to provide assistance to the educator. The young female is not an EEC approved caregiver at the time. She has an application in process to become a Regular Assistant.
Licensor observed a cigarette butt floating in the bathroom toilet.
The educator is using two unapproved caregivers to staff the program on 11//12/19. The educator reported that those two unapproved caregivers are the only staff working with her.
The daily attendance is not being documented accurately. The attendance record for the current week 11/11/19-11/15/19 is being pre filled with the children’s schedule instead of using accurate arrival and departure times.
The educator did not demonstrate good judgement when she allowed unapproved caregivers to assist her in caring for 10 children, including {REDACTED}.
Licensor observed 2 children napping inside of a room with the door closed.
The educator left 10 children unattended in the basement with an unapproved caregiver when she opened the door for this Licensor at 2:20pm at the kitchen on the first floor.
Licensor observed the following outdoor safety hazards: 1. Uncovered grill and 2 tanks of propane gas. Licensor observed the following indoor safety hazards: 1. Uncovered trash container in the bathroom.
1. Licensor observed that the educator was using a recalled “infant rock n’ play sleeper.” 2. The educator reported that she rotates {REDACTED} among bassinet and playpens available in the program.
