Licensors observed an unlocked drawer in the kitchen area where educators stores plastic bags and sharp utensils.
At the time of the visit a random record review was done and the following items were missing from the children’s records: Child 1: Missing the annul update form, missing assistants names on permission forms and emergency card. Child 2: Incomplete physical description, missing assistants names on permission forms and emergency card. Child 3: Missing anticipated times of attendance, missing assistants names on permission forms and emergency card, missing lead test, physical exam, and vaccinations. Child 4: Missing enrollment packet, missing lead test, physical exam, and vaccinations.
