At the time of the visit EEC observed the following at the {REDACTED} program: a) One IHCP for a child with a diagnosis of a chronic medical condition was not available for review. b) One medication listed on a child's IHCP was not available for review.

At the time of the visit EEC observed the following: a) A current staff records checklist for the {REDACTED} Program was not available for review. b) Staff members files of the {REDACTED} Program educators were missing required documents.
At the time of the visit one educator at the {REDACTED} Program did not have a completed BRC.
At the time of the visit, two cleaning products (All Purpose Cleaning Wipes) were stored in an unlocked cabinet in a {REDACTED} classroom.

A review of six staff records reflected the following. 1. Documentation of {REDACTED} immunizations were not available for review for one staff member. 2. First Aid Certifications were not available for review for two staff members hired past their six months of employment. 3. Documentation of verification of two references and interview were not available for review for a staff member. 4. An updated physical examination was not available for review for a staff member.

This monitoring visit inspection recorded no violations or advisories.

Documentation of {REDACTED} staff meetings held between the months of June 2022 through September 2022 were not available for review. A review of October 2022 {REDACTED} staff meeting did not include documentation of the minimum two hours.
Administration
Equipment is clean, safe, secured, and poses no hazard.
The maximum group size was exceeded during the use of the gymnasium which reflected of 28 {REDACTED} children freely playing.
At the time of the visit, electrical cords were not safely arranged in two {REDACTED} classrooms and in the gymnasium.
A {REDACTED} classroom licensed for 15 children exceeded the maximum capacity when two {REDACTED} groups combined, totaling more than the allowable licensed capacity; space did not meet the 35 square feet of activity space per child.
At the time of the visit, an administrative staff was found to be in ratio full time and reported this to be a daily occurrence due to insufficient staffing.
A child's Individual Health Care Plan for an {REDACTED} medication did not contain parental consent.
The Building Inspection posted on site and submitted for review reflected expiration date of 11/30/2022.
