Licensor observed that two children were missing assistant’s names on permission to transport to medical facility and receive emergency medical treatment.
Licensor observed that the program's first aid kit was missing a gauze roller.
After the review of five random children’s records, Licensor observed the following: 1) a child was missing the child’s physical description in the enrollment packet. 2) two children were missing assistants information on the General permission and CPR and FA medical consent and release in the enrollment packet. 3) three children were missing up to date emergency cards.

Licensor observed the following indoor health risks accessible to the children: 1) a white extension cord with a plugged in phone charger in the first floor living room licensed space, which presented a strangulation hazard for children. 2) an orange extension cord in the first floor presenting a tripping and strangulation hazard for children.
