One child's Individual Health Care Plan did not include who had been trained and will be administering this treatment while the child is at the program.

In the Infant Classroom, there were 5 children in attendance. A teacher left the classroom to put items in the children's cubbies (hallway), leaving the Program Director alone with 5 children. Staff to child ratios must be maintained at all times.

In the Toddler Classroom, an infant bottle was observed on a child's table and was not labelled with the child's name it was intended for.

During the visit, the barrier was not up between the toddler classroom and the office, to prevent the children from accessing the office area.
Written progress reports were not prepared for infant children attending the program. A written progress report must be prepared every three months for infants and children with identified special needs enrolled in the program.
Documented observations of educators while working with children were not conducted. Observations must be conducted at least every two months by a staff with lead teacher, site coordinator or higher qualifications
The allergy list posted in the Toddler Classroom was not posted in a manner that protected the privacy of the children.
One child's epi pen medication was stored in a blue case and not in the original package with the prescription label affixed. This epi-pen expired on 8/2024. The Director did have a new epi-pen on the premises. However, the expired epi-pen should have been removed from the medication bag and returned to the parent.
