One of the children was not on the same floor level as the educator. The child was in the basement with another child household member while the educator was on the first floor.

One child’s enrollment packet has several sections which are not fully completed. One child needed an updated annual form and a physical.
There was minimal evidence of a developmentally appropriate set up of the space and activities for all aged children enrolled. The living room area does not have activities set up for the children.

The attendance log did not accurately reflect all of the children who were present.
The educator and two household members have expired background record checks. The educator also has a child household member who is not listed in the LEAD portal.
The diapering mat the educator is using is ripped and not in good condition.
The water temperature measured 130 degrees over the 120 degrees maximum.
There is one child with a medication allergy. The educator does not have the required posting.
The educator was not actively engaged in the supervision of one of the children enrolled in the program. The child was in the basement space while the educator was on the first floor caring for the other children.
The educator provided false and misleading information to the Department of Early Education and Care (EEC). The educator was asked at the start of the visit how many children were present. The educator stated only the four children which were present in the play room. The educator reported one child did not attend today. During a fire alarm check, the licensor opened the basement door and two children were observed at the bottom of the stairway in the basement. The children were instructed to come into the licensed space. The educator stated she had forgotten when asked about how many children were present and over looked the two children who were in the unlicensed basement space.
The educator did not use good judgement in allowing one of the child care children to be unsupervised in unlicensed space with another child household member.
In the kitchen several of the drawer locks were not functioning allowing access to sharp knives, can openers, wires, bags, batteries, scissors and a screw driver. In the bathroom, a drawer was not locked allowing access to wires bags and razors.
There were two children in unlicensed basement space. The two children included a ten year old child and a thirteen year old house hold member.
All of the children’s progress reports were reviewed, three children require an updated progress report.

All of the children’s records were reviewed, one child did not have documentation of their lead testing, one child did not have a physical on file and one child did not have an annual update form.

The first aid kit was missing the children’s emergency cards.
The educator went outside with one of the children to play while one child was left sleeping inside with a household member.
This follow-up visit inspection recorded no violations or advisories.

In the kitchen area, there were safety locks on the cabinet doors but none were in use allowing access to several hazards to include but not limited to a lighter, batteries, a blender, cheese grater, cleaning products and sharp knives. In the bathroom there were cleaning materials, personal hygiene materials accessible to the children.
There were electrical outlets in the living room and the kitchen which were uncovered. The was a fireplace with sharp stone edges accessible to children in the living room.
A bottle warmer was observed in the kitchen to warm a bottle.
Provider was observed to change a child's diaper, provider did not utilize two diapering surfaces either able to be disinfected or disposed of during the diaper change. Provider was observed to not wash her hands or the child's hands.
Provider did not have sufficient materials for children of all ages.
