At the time of the visit the program had the following equipment in need of repair: the blinds in the {REDACTED} and {REDACTED} Room, two mats in the {REDACTED} classroom have holes and one is potential tripping hazard.
At the time of the visit the program did not have the following evidence in the staff records: two missing interviews, and one missing first aid certification,

At the time of the visit the program had one expired IHCP.
At the time of the visit the program needs to fix the following items on the playgrounds: Preschool: the outlet near the {REDACTED} ramp is missing a cover, the back of the ramp is missing the plastic piece to prevent anything from going under, the picnic table is not secured and lifting up, the easy up tent is not secured on one of the legs and the children can move the leg, and two screws on the fence are sticking out. On the {REDACTED}: the wooden box cover over the pipes has screws sticking out and the side is not secured, there is a wire that needs to be secured to the side of the building, and the plastic cover over the window well is not sturdy as the children try to climb on top of it.

At the time of the visit the {REDACTED} classroom had 12 children signed in however 14 children were present.
At the time of the visit the educator in the infant/toddler classroom was not using positive child guidance with a young child when they told the child they would put them in the jumper if they did not stop.
At the time of the visit the program did not have the expiration dates on the crackers in the container.
At the time of the visit the program had several areas in the {REDACTED} classroom and {REDACTED} classroom with peeling and/or chipping paint.
At the time of the visit the program had two bottles of medication expired. One bottle was antihistamine and the second bottle of medication was for a child in case they ingested fish.

The submitted staff schedule indicated that an assistant was alone in the {REDACTED}room from 6-6:30pm. It also shows that 2 assistants work together from 3:30-6pm in the {REDACTED}l room without a teacher.
One Educator did not have documentation of 2 reference checks.
The fire drill log did not contain all of the required information including the number of children evacuated, which exit was used, and the effectiveness of the drill.
This investigation inspection recorded no violations or advisories.
