Playroom: white toy shelf not secure. Kitchen: lighteners, cork screw, vegatable peeler in unlocked drawer, plastic bags, aluminum foil box with serrated edges in unlocked drawer, plastic bags and chocables in unlocked drawer, chopper and grater in unlocked cabinet. Playroom: one rusted floor vent cover and one damaged floor vent cover Dining room: two rusted floor vent covers Outdoor space: uncovered sandbox, fence slats on grass, gate does not have a latch to secure door in place.

The educator lives in the corner of a busy street. Educator installed vinyl privacy fencing all around the property, however there is one drop off wall that leads to parking spots near the garage and one area with rocks in the back yard. Both areas present fall hazards and must be barricaded. Discussed options during the visit. Please respond with the option chosen and send pictures with your response.

The educator was operating above her licensed capacity at the time of the visit. Seven children were present including her own two children.

Child 2 - missing physical description Child 3 - Missing anticipated dates and times of attendance, general permission (transportation) is missing destination and means of transportation, doctor’s information Child 4 - missing physical description, missing annual update form, missing updated medical with vaccines and lead screening Child 5 - missing physical description, missing location and means of transportation under general permissions.
