The Epi-Pen reviewed expired in June 2023. The prescription does not expire until April of 2024.
IHCP plan was missing who was trained. Another IHCP plan did not have any instructions on when to administer the medication. Epi-Pen and Benadryl.
Staff Educator Schedule was not current, staff first aid and CPR expired on the scheduled.
The program has documentation and a log from May 2021. The director reported she does not write injury reports for minor injuries, But does communicate with the families verbally.
Both emergency medications reviewed were missing the prescription label.

Outdoor Space - R Apr 2025
During the visit children were observed in areas that were not clearly visible to educators. Creating supervision concerns.
The fence was broken leaving a gap that would be a potential head entrapment. The sand in the fall zones was compacted and hard.
Staff members were missing evidence of MMR's one was missing evidence on both the other was missing one of the required two. Staff files were missing BRC information.
Children did not wash hands before playing in the sand table in the downstairs preschool classroom.
At the time of the visit it was observed that some of the space could use a more thorough cleaning. Children's chairs, floors, walls both kitchen spaces and the cabinets.

Program director reported they have not been doing 2 hour staff meeting after hours, they have just been talking on the playground.
One staff file missing interview, application, references, evaluation, current physical and no documented professional development for the current year.
Current staff records checklist was not available during the visit.
Nurse practitioner has an expired certification date.
Bathroom trash has no cover.
The monkey bars and the dome have chipping paint. Hazardous projection on the metal slide, the screw extended more than two threads from the bolt. The Little tikes climber was broken and leaning. The fence was not secured into the ground, it was leaning against the existing fence and held together with cords. The fence has slits that are head entrapments.
