During a review of children's records, it was found that the enrollment paperwork was expired for at least four children.
The fall zones under and around the climbing structure in the outdoor space do not have an adequate depth of 9 inches in three out of five areas. The fall zones under and around two swings do not have a mat or an adequate depth of 9 inches.
Fire drills were not conducted for September and October.

Staff files were not available for review during the visit. The Staff Records Checklist was not available for review during the visit. Evidence of a completed BRC for the one educator that works at the program was not available during the visit.
During the visit there was not a Program Administrator or Site Coordinator on site, The designated person in charge is an Assistant Group Leader and has been alone with 6 children for two weeks. It was reported the new Site Coordinator will start at the program next week.
One child’s file had an expired transportation plan, First Aid consent, IHCP and had a missing progress report. Another child’s file was missing a First Aid consent form and progress report.
Staff records were not on site for review. Staff Records Checklist was not onsite for review.
The table was not washed prior to children sitting for snack. Children did not wash their hands before or after snack.
[Redacted] reported they have not done any evacuation drills since November when they started. No evacuation drills were documented on the Evacuation Drill List.
[Redacted] was not sure if the children on the Allergy/Special Conditions list were enrolled. They were knowledge able about one of the children who had a gluten allergy.
When the children came into the program they were walking around swearing and speaking inappropriately, The Assistant Group Leader did not address them, when they were closest to the child. One child climbed on top of the shelf, when the site Coordinator who was across the room told them to get down, the child was not redirected. The Assistant Group Leader was within five feet of the child climbing and did not intervene. There was no structure when the children came into the program, they all just went everywhere and had no direction. The were running across the room, hitting the wall and using inappropriate language.

One staff file was missing, staff form, program orientation, Essentials Training, Bi-monthly observations, PQ Registry, physical and immunizations. Another staff file was missing a physical bi-monthly observations, annual evaluation, proof of MMR and a current PQ registry.
A child prescribed Benadryl had expired Benadryl as of 01/2024. Medication Consent forms signed by the parent for Benadryl, Albuterol and Epi-Pen were missing.

During activity time the Site Coordinator was doing an activity with a group of children and speaking to the Licensor, when a child went to exit the door to outside, the Site Coordinator stopped the child, then the child left the classroom into the hallway and proceeded down the hall way. During this time the Assistant Group Leader was sitting at the table coloring a picture, not engaged with the children or taking over the group of children while the Site Coordinator was tying to catch and work with the child who left the classroom alone.
The Staff/ Educator Schedule was not posted or available during the visit.
The snack tables were not washed prior to serving the children snack.
There were food crumbs on the table with a toy bucket on it. Box’s and plastic bins were stacked in the corner of the room near the exit. Bins, boxes and teacher supplies were stacked and leaning on the counter top.
