The outdoor play space was observed, and the following was noted: 1. The yellow plastic coating on the stairs has separated from the metal, it is protruding out with sharp edges and the metal is now exposed. 2. The red paint has chipped/peeled exposing the metal which is beginning to rust. 3. The paint has peeled off of the yellow bars across the roof leaving the metal exposed which rusted.

4 staff files were reviewed and the following was noted: 1. Date of hire was missing from 2 files. 2. Yearly staff evaluations was missing from 3 files. 3. Current physical exam was missing from 2 files. 4. Program orientation was missing from 3 files. 5. updated registration in the PQ registry was missing from 3 files. 6. Evidence of the EEC 12 Essential trainings was missing from 1 file.
