3 children's files were reviewed, and the following was noted: 1. All files were missing the date of admission. 2. The 1st Aid emergency consent and release form in Procare was not available to view or print, it shows only a check box that it was completed, this must be viewable/printable in an event that an ambulance needs to transport a child to the hospital. 3. 1 3-year-old child was missing evidence of a 3-year-old lead screening.

When reviewing IHCP's the following was noted: 1. 1 child with an [REDACTED]did not have an IHCP on file. 2. 1 child's IHCP was missing required information, it did not state which Educators received training in that child's IHCP. 3. 1 child's IHCP was not signed/dated by the parent.

When observing in classrooms the following was noted: 1. 2 bottles of hand sanitizer were found stored on low shelves accessible to children in the Pre-K classroom. 2. Disinfectant deodorant spray and Aqua steel metal polish were found stored in the children's bathroom within reach of children.

When observing in classrooms the following was noted: 1. The kitchen sets in in the PS and Toddler rooms were not secured. 2. The top of a blue plastic trash/recycling bin was cracked, sharp edges were accessible to children.
At the time of the visit a [REDACTED]that was attached to his body, the program was administering [REDACTED] needed throughout the day, there was no medication log on file of administering the medication.

When observing the playground the following was noted: 1. A plastic red/blue slide did not have the required 6 foot use/fall zone completely surrounding it and there was no impact absorbing material underneath it as required. 2. Trash, coffee cups/debris was found in one corner of the playground.

At the time of the visit the Toddler classroom had a basket and bag full of toys on top of the diaper changing table, this surface cannot be used for anything other than it's intended purpose.

3 children's files were reviewed, and the following was noted: 1. 1 file was missing evidence of an updated lead screening. 2. 1 file was missing evidence of an updated physical exam.
3 staff files were reviewed, and the following was noted: 1. 1 file was missing a staff information form, and the date of hire was incorrect as the staff member left employment and then returned. 2. 1 file was missing the documentation of interview date, an updated physical exam and evidence of [REDACTED].
A small brown table was not secured and was tippable in the Toddler room.
Written emergency evacuation procedures were not posted by the back door of the Pre-K classroom and the main door to the Toddler room.
At the time of the visit the following was noted: 1. Children in the Pre-K classroom were instructed to get a tissue but did not wash their hands after blowing their nose. 2. Pre-K Tables were not washed with soap/water prior to sanitizing them. 3. The diaper changing table was not washed with soap/water and then disinfected. The Educator sprayed both on the surface together and then wiped clean.

Shelves in the Toddler room were covered with sheets making the toys/equipment not accessible to the children.
At the time of the visit an exercise trampoline was located in the gross motor room accessible to the children.
1 child had an IHCP on file however it did not contain all of the required EEC information and did not have the parent's signature and date.
Children in the PS room did not wash their hands prior to eating snack.
[REDACTED] with 2 chronic medical conditions an [REDACTED] did not have an IHCP on file for either condition.
At the time of the visit the Licensor noted the following: 1. An Educator's purse was accessible to children in the Pre-K and Toddler classroom. 2. A bottle of hand sanitizer was accessible to children in the Pre-K classroom.
When observing the Toddler and Pre-K classroom the following was noted: 1. The diaper changing surface was not washed with soap/water and disinfected in between changing the Toddler's diapers. 2. The Toddler Educator did not wash her hands after cleaning the diapering surface. 3.The tables in the Pre-K classroom were not sanitized after washing with soap/water prior to the children eating snack.
