When measuring new space during the Change in Space visit the following was noted: 1. Window guards were not yet installed for the new upper level Pre-K classroom. 2. Cubbies and the dramatic play kitchen set were not yet secured to prevent tipping.

3 children's files were reviewed and the following was noted: 1. 3 files were missing the date of admission. 2. 1 file was missing evidence of a current physical exam.
Through a review of the evacuation log it was learned a fire evacuation drill was not conducted in June 2025.
3 staff files were reviewed and the following was noted: 1. 1 file was missing evidence of an updated physical exam (expired 8/23), did not have evidence of updating in the PQ registry, and there was no evidence of professional training hours for 2024. 2. 1 file did not have evidence of a yearly evaluation, evidence of having completed the EEC 2.0 Essentials training by June 30th, 2025, and there was no evidence of professional development for 2024.

3 children's files were reviewed, and the following was noted: 1. Date of admission was missing from all files. 2. 2 files were missing evidence of a physical exam. 3. 1 file was missing evidence of immunizations.
Upon arrival at the program the upstairs PK classroom Educator had all 3 classrooms open for children to use. The Educator was alone and could not appropriately supervise all 3 rooms due to it being an "L" shape. The Educator was in the back room and could not see the 2 children in the front side room near the kitchen.
When arriving at the program the updstairs Pre-K classroom was over the teacher to child ratio, one Educator was alone with 13 children, the Licensor walked in with the second Educator at 9:08 AM.
At the time of the visit there was no evidence on file of yearly reviews for Educators.
6 staff files were reviewed, and the following was noted: 1. 1 staff member did not have evidence of professional development hours.
