Educators in the Twaddler 3 classroom were overheard yelling at children. One educator was yelling at a new student who was crying, "Yes, your mother is coming, now stop crying!" Another educator repeatedly yelled at children to stop touching the paint. The second educator yelled at a child who was reaching towards the diaper changing table and attempted to swat the child's hand away.

In multiple classrooms there were empty cubbies leaving a limited variety of materials for children to engage with.
During observations in 2 of the 6 classrooms educators were observed using only a 1 step process to clean and disinfect after diapering.
Not all sippy cups were found labeled for the child it was intended for.
During health and safety inspection the following was found: peeling paint, dirty carpets, holes in walls and a piece of a cubby was broken off.

A review of the attendance in Infant A showed six children signed in while there were 7 children present.
The staff schedule was not posted in an area visible to parents and visitors.
The changing pad in the Twaddler 3 classroom was cracking on the corners.
A review of three IHCPs showed one to be expired and all three were missing the required training information.
Review of 5 children's files found the following: 1 child's files was all outdated and their first aid/cpr consent was never signed or dated and another child's files had a transportation plan and developmental history form signed but not dated and did not contain a current physical.

Multiple staff had their personal drinks stored within reach of children.
During the visit staff observation documentation could not be located therefore it could not be determined if observations are being conducted of all staff at least every other month.
During the visit the evacuation drill log could not be located therefore it could not be determined if evacuation drills have been completed monthly.
An emergency medical bag was found in one classroom but belonged to another classroom. Staff were unaware the bag containing emergency medication was missing.
