Individual Health Care Plan (IHCP)
One [redacted] was missing the original box and the prescription label. The medication consent form said repeat [redacted] after ten minutes if needed, there was only one [redacted].
While reviewing an Individualized Healthcare Plan (IHCP) it was missing the parent signature. Another IHCP reviewed was expired.

Medication
While talking to the educators about children listed on the posted Allergy list. The educators were unsure what child had diabetes and had insulin at mealtimes and had medication onsite. Another child had an allergy and was prescribed an epi-pen and Benadryl. The educators reported they did not know who had the Benadryl prescribed.
Two children with prescribed medication were missing a current completed medication consent form signed by the parent.
During the file review educators did not have evidence of a completed BRC. Some educators had the consent form for the BRC.
The program had several children with life saving medications that were not listed on the Allergies/medical info sheet.

Two children with an IHCP and prescribed medication were missing parental consent for administration of medication. A child with a Peanut allergy was missing required information on the Individualized Healthcare Plan.

A medication consent form for Novolog Insulin was missing parental consent. A medication consent form for an epi-pen was missing parental consent. A bottle of Benadryl was open in a zip lock bag, the bottle did not have a child's name on it.
