In the outdoor space, one picket is missing on the wood fence and it poses a head entrapment hazard and in another section of fence, a board is missing on top of the fence and there are three protruding screws.

During a review of staff records, it was found that three files were missing required documentation.
The attendance records were not accurate in one classroom; one child was not signed into the preschool room and the attendance records were in the lobby, not in the classrooms with the children.
Upon inspection of children's medications, one OTC medication was expired.
During a review of children's medication consent forms, it was found that one child was missing doctor's consent for an over-the-counter medication.
