Individual Health Care Plan (IHCP)
At the time of the visit, within the Preschool One classroom, it was observed that one preschool student was permitted to use the bathroom unsupervised. The student bathroom was located down a hallway, outside of the preschool classroom space. Educators must be positioned to maximize their ability to see and/or hear children in their care. When this was identified by EEC, one the Licensees and the Assistant Teacher in the classroom immediately went to the bathroom space to ensure proper supervision for the rest of the time the student was in the bathroom.
At the time of the visit, in a review of three students IHCPs, the documentation was not fully complete. Student B's doctor provided IHCP did not provide a diagnosis. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

At the time of the visit, it was observed that a play kitchen within a Preschool Classroom was not secured and was tippable. The licensee must ensure indoor and outdoor equipment is sturdy, safely constructed and installed and non-tippable.
At the time of the visit, in a review of three IHCPs and the corresponding medication consent documentation, the information was not fully complete. One students medication consent document was missing the directions for storage.

There was not a current Individual Health Care Plan for Child S, diagnosed with (REDACTED).
There was not a current parental consent for administration of (REDACTED) for Child L, (REDACTED) for Child Z, and (REDACTED) for Child S.
During the visit, when the exit door onto the playground was opened, the door fell completely off the frame/hinges, and fell directly onto the playground. The playground was in use at this time; no children or staff were injured. The disrepair of this exit door frame/hinges resulted in a safety hazard to children and staff. Staff stated that the previous evening, at closing time, it was noted that the "door had a few screws fall out of the molding of the door on the top two hinges (most of the screws stayed in the hinges - the picture (attached in email) will show that the screws were still in the hinge but the hinge came unattached from the molding of the door." The closing staff "indicated that it was not closing properly. (She) immediately made a sign to not use the door". This was reported to the co-owner/Director that evening. However, the message was not relayed to the co-owner/Director who opened the program the following day. Additionally, staff stated "We have expressed to our landlord several times in the past that we needed a new door, just because it was older and we were concerned mainly with heat loss, but certainly never for anyone's safety."

The program did not have evidence of observation of educators while working with children at least every two months by a staff with lead teacher, site coordinator or higher qualifications. In addition, the file reviewed did not have a current PQ registry.

The following were observed: ---Tippable furniture (Room 1, hallway, Room 7) ---Cords over 6 inches and accessible to children (Rooms 1, 2, 6, 7) ---Rugs that were curling or able to slip/ slide (Rooms 1, 4, 7) ---vinyl climbing gymnastics cubes next to shelf and on tile floor. ---Potential issues with evacuation (stacked books on rolling shelf in hallway, broom leaning in corner at hallway to bathroom rim 7)

Toxic Substances
A teacher purse was accessible to children in Room 3 and two purses in Room 9. In Room 5 a bag full of plastic bags were accessible to children in an unlocked cabinet. These hazards were removed during the visit. A water cooler, approximately 4 feet tall, unsecured, was accessible to children in a hallway.

The program did not have evidence of all monthly staff meetings. The program had documentation of meetings in 2022 for April, July and November.
A review of one child's file indicated that the parent had not signed or dated the annual enrollment form.
In one room, an educator reported that they clean the tables prior to meals, but only sanitize or disinfect at the end of the day. When specifically asked if they sanitize between meals or only at the end of the day, the educator replied they only sanitize at the end of the day.
