-At the time of the visit, the adult desk and two drawer filing cabinet in the entry way of the cafeteria each had visible rust and chipped coating that is in need of repair or replacement. -The drawers to the two drawer filing cabinet were observed to have difficulty closing and locking. This is where confidential information is kept and this must be locked and secured.

A review of medication stored at the program indicated the following: -One child's prescribed (REDACTED) could not be located during the visit. This means in the event a potential emergency, educators did not have a life saving medication that was prescribed to a child readily accessible to administer.
A review of Medication Consent Forms for children enrolled at the program who have been prescribed medications indicated the following: -One Medication Consent Form listed two separate medications (REDACTED). Each medication must have its own individual Medication Consent Form.

A review of five children’s files indicated the following: -Five did not have evidence of Lead screening. -Two did not have evidence of a current physical examination. *For school age children, a written parental statement that the required physical examination, lead screening, and immunization information is on file with the child’s school is sufficient.
