A review of six staff records indicated that one did not contain evidence of completing the EEC Essentials 2.0 training.

A review of the staff meeting log from January 2023-November 2025 indicated the following: 1) Five did not have the meeting time documented; 2) Two did not document the meeting date, only the month; and 3) There was no documentation of a meeting held in May 2023.
It was observed in the GYM that some of the vinyl border panels were cracked or loose.
It was observed in the GYM that there was chipped paint, exposing the cement wall.
A review of four IHCPs indicated that two did not identify which staff were trained and designated to administer medication (this was corrected immediately during the visit) and one had conflicting information between the doctor provided documentation and the form filled out by the child's parent.
A review of six staff records indicated that two did not have evidence of MMR immunization records and one did not have evidence of a program orientation completed.

Medication
The following was observed and accessible to children: In ROOM 1, there was a stereo cord unsecured in the circle area. There was a loose panel in the HALLWAY underneath the drinking fountain. In ROOM 2, there was an uncovered outlet underneath the bulletin board and a toilet brush located in the children's bathroom.
A review of four IHCPs indicated the following: A secondary medication listed on a doctor's medication order was not available at the program, one written parental consent was not signed by the parent, two secondary medications did not have a written parent consent, and all four did not identify designated trained staff to administer medication.
Indoor Space
The program has sand under equipment that has a fall zone over four feet. Any fall zone over four feet requires one of the following: shredded/recycled rubber, wood mulch, or wood chips. Wood chips cannot be used for infants and toddlers.

This renewal - full review visit inspection recorded no violations or advisories.
