The records were reviewed for the 5 enrolled children. One child did not have proof of an enrollment packet and a lead screening. Another child did not have proof of a physical, immunizations, and lead screening.

There was no thermometer in the refrigerator. Please send proof of the refrigerator temperature. Discussed the temperature needs to be between 32 and 40 degrees.
There was a plastic sand box in disrepair and a piece of lattice laying on the ground in the outdoor play space. There was also a dog excrement removal device accessible to the children in the outdoor play space.
The Educator did not have the required emergency posting. Please send proof of posting.

In the bathroom, there was plastic from packs of diapers accessible to the children. Packages of diaper wipes were also accessible to the children. Two diaper packages and 1 wipes package were observed on the shelf of the changing table and 9 packages of diapers in plastic under the sink. There was a mop and bucket with water in the tub in the bathroom accessible to the children. There was toilet cleaning brush behind the toilet. In the playroom, there was a small plastic sandwich-size bag and food wrappers accessible to the children. In the bedroom, there was a shelf that was a tipping hazard and a cord (longer than 6 inches) from a window shade that was accessible to children when they are in the crib.
The Educator brought 2 children into the kitchen with her; the kitchen is unlicensed space.

The Educator had two toddlers, (REDACTED), in exersaucer type devices. These devices are not intended for use with children who can walk independently; both children could walk on their own. The Educator compromised the safety of the products and the children by not using them according to the directions.
Interactions with Children
Several bottles and sippy-type cups were observed. None of the bottles were labeled with the children's names, as required by EEC regulations. Other children had access to the bottles and sippy cups. Children's coats and backpacks were piled on top of each other. There was not sufficient space for each child to store these personal items in a safe, sanitary manner.
EEC Licensor observed the door to be shut to the bedroom where a child had been sleeping, per Educator report. EEC Licensor could hear a child crying, so she opened the door. A child, approximately (REDACTED) was observed standing up in crib and crying.
EEC Licensor observed that there was not a distance of two feet between three cribs located in a licensed bedroom as required.
Licensed Space
During the visit, two toddler children were observed to be confined to exersaucer seating devices for approximately 60 minutes. An infant child was observed to be confined to an infant car seat for approximately 60 minutes. EEC staff requested during the visit that the Educator remove the infant child from the car seat. The Educator also used cribs to contain one toddler for approximately 30 minutes. It was not observed that the infants and toddlers had opportunities to move freely and achieve mastery of their bodies through self-initiated movement, including opportunities to practice emerging skills in coordination, movement, balance, and perceptual-motor integration.

The Educator admitted to having an unapproved person on the premises regularly. This person was not present on the day of the visit, but the Educator reported the person had been there until recently.
The provider’s licensed capacity is eight children with a condition that two of the children must be school-aged. On the day of the visit, the Educator was caring for seven children ages 5 months, 11 months, 16 months, 22 months, 2, years old, and two children that were three years old. At no time shall a program admit, supervise or provide care for more than the maximum number of children indicated on the license.
A review of seven children’s records indicated that the last progress reports were completed in December 2022 for at least two children. One other child had been enrolled in the program for more than six months; there was no evidence of a progress report for that child. The Educator did not complete current progress reports for the children enrolled in the program as required.
The Educator was observed during the visit napping two children in the same crib without changing the sleeping materials. Additionally, one crib was observed to have chipping paint around the edges.
The Educator reported she had an “aid” helping her in the program. All Educators must be approved by EEC before caring for children in licensed programs.
There was not a sufficient variety and amount of developmentally appropriate play materials accessible to the children.
During the visit, the Educator was observed preparing lunch in her kitchen while a (REDACTED) was in a car seat located around the corner in a hallway. Children younger than six months of age at the time of enrollment must be under direct visual supervision at all times, including while napping, during the first six weeks they are in care. Additionally, a 22-month-old and 16-month-old were seated in exersaucer seating devices at the end of the hallway in bedroom while the Educator prepared lunch. Educators must be positioned to maximize their ability to see and/or hear children in their care. Additionally, Educators must be in sufficient proximity to children at all times in order to be able to intervene quickly when necessary.
The Educator was not responsive to individual children's needs. Children were observed crying and were confined. The Educator failed to recognize signs of stress in children’s behavior and responding with appropriate stress-reducing activities. The Educator confined children to cribs, exersacuser-type equipment, and infant car seats for an extended period of time in lieu of supervision.
Uncovered and unused outlets were observed in all rooms of the licensed space. These outlets were accessible to children.
Records were reviewed for the seven enrolled children. Two children needed proof of current medical documentation. Two children needed Annual Update Forms.
Children were not offered alternative activities in lieu of napping.
On the day of the visit, the Educator was caring for seven children. Four of the children were ages 5 months, 11 months, 16 months, and 21 months. The Educator cannot care for more than 3 children under two years of age. EEC licensing staff requested that the Educator come into compliance with this regulation during the visit as it impacted the children’s health and safety. While EEC staff were present, the parent of the (REDACTED) to pick up her child. Please submit your enrollment plans to EEC to come into compliance with this regulation.
During the visit, the Educator had an attendance record sheet for the week of 11/27/2023 completed however, there were no other attendance records.
There was a shelf unit in the playroom that was broken and accessible to the children. There were electrical cords from various devices accessible to the children in every room of the licensed space. In the playroom there was a cable wire from the television was hanging and accessible to children. There was(REDACTED) in the side pocket of a child's lunch box, which was accessible to children.
During the visit, the Educator did not provide any evidence of a plan for the day, including program activities and engaging learning experiences. EEC staff did not observe that there had been opportunities for children to engage in a variety of creative activities, such as art, music, literature, dramatic play, and science, which encourage exploration, experimentation, and discovery. EEC staff observed three children jumping on cots and watching television during the scheduled rest time. Additionally, the Educator stated that the last time the children were taken outdoors was during the summer months, and there was no evidence that the children participate in 60 minutes of physical activity per day as required by the licensing standards.

Progress reports were reviewed for the three enrolled children. Two had progress reports that were overdue. Discussed the frequency of progress reports based on the ages of the children. Also discussed the progress reports must be shared with parents.

The Educator did not have proof of the required ongoing professional development training hours. Discussed the Educator must have 10 hours of professional development training annually, including 1/3 of the hours being in the diverse learners category. Please document a plan for completion of the 30 hours and log the trainings in the PQ Registry.

The Educator reports she has not been completing the evacuation drills, as required by EEC regulations. Discussed the evacuation drills must be completed monthly from alternating exits and documented on the evacuation drill log .
The Educator gave the children a snack, but did not wash her hands or their hands before giving it to them.
The Educator did not have the vaccination records nor the the license for the dog available for review during the visit. Please send proof of vaccinations and licensure for the dog.

The records were reviewed again for the enrolled children. The one child still had an incomplete enrollment packet. Discussed the Educator needs documentation of a lead screening for the other child. If the child has not had a lead screen yet, there needs to be a plan in place to get the child screened.
One infant was found sleeping in an infant swing. The infant was sleeping at a slight incline in the swing, not flat on her back as required by EEC regulations. This is the Educator’s second Safe Sleep violation.

CPR and First Aid both expired on 1/3/2022. Discussed that the Educator must close her program at the end of the day and remain closed until she has renewed her CPR and First Aid certification.
The Educator does not have any professional development training hours documented in the PQ Registry. The requirement is at least 10 professional development training hours annually, with 1/3 of the hours being in the category of diverse learners.
The (REDACTED) and both have been enrolled less than 6 weeks were in separate rooms with the doors slightly ajar. The Educator was unable to have both (REDACTED) in her direct visual supervision at all times.
The Educator did not have the infants napping in appropriate cribs. The Educator has one crib and one porta crib. EEC Licensor discussed that the toddler could sleep on a mat and the infants need to sleep in the crib and porta crib.
EEC Licensor observed 2 infants sleeping in infant swings when she arrived. One infant was wearing a bib and had a pacifier that was attached to a clip ribbon. The other infant was swaddled, not allowing free movement of the infant’s arms.
