Evacuation procedures were not posted in the Toddler classroom as required.
This follow-up visit inspection recorded no violations or advisories.
During this visit there were two unsafe sleep violations observed in the infant classroom: A (REDACTED) was sleeping in the crib with a binky attached to a covered elastic with a clip on the end. The same child's bib was also hanging in the crib. There is a paper towel holder on the wall next to the crib; a paper towel was hanging low enough for the baby girl to grab it from the crib. All three of these are unsafe sleep violations. A (REDACTED) was sleeping in the crib with a binky attached to covered elastic with a clip on the end. The sheets in the crib were very bunched up and the sheet did not fit the mattress. Both of these are unsafe sleep violations.
-One Individual Health Care Plan did not have a signature by the doctor or a date. The same plan was expired. 7/16/24 -One Individual Health Care Plan expired on 5/15/25. -One Individual Health Care Plan expired on 4/11/24. All of these plans are required to be updated annually.

The upstairs preschool classroom's attendance had documented there were 8 children present. There were 10 preschool children present in the classroom on 9/13/23. Programs must maintain accurate attendance records. A method of knowing exactly who is present on the premises at any given point in the day.

Current consent for administered medications
Space (indoor / outdoor hazards)
The Infant/Toddler and Preschool 3 classrooms had cords hanging loosely: more than 6 inches in length. This is a choking hazard for children. -In the toddler/preschool classroom the thumbtacks were accessible to the children.
Three medication consent forms were expired: 4/11/25, 5/15/25, and 6/20/25. All medication consent forms must be updated annually.
The exit door in the downstairs preschool classroom was open. There were no children in the classroom. The door has direct access to the street.

In the preschool playground under the red slide there was approximately 3 inches of impact absorbing material. Nine inches is the adequate depth for fall zones.
The licensee did not maintain the outdoor play area as it had unknown vegetation growing into the playground accessible to children.
The licensee did not hold evacuation drills during the months of August and September 2020. The licensee must hold monthly practice evacuation drills.
The licensee did not document who was trained to administer medication and who trained the individuals to administer the medication.
-The licensee did not maintain current Building, Fire, and Health inspections as required. -The IPM plan did not have any documentation of a current date.
