The following was observed and accessible to children: 1) In ROOM 4 and ROOM 5, there was a Magic Eraser cleaning pad located on the handwashing sink. 2) In the LEFT CHILDREN'S BATHROOM, there was an open box of gloves located on the low window sill.

Individual Health Care Plan (IHCP)
BRCs on all potential employees
A review of medication consents for five IHCPs indicated the following: 1) One did not include the dates and times for the medication to be given and directions for storage. 2) One did not have evidence of consent for an emergency medication and was missing a parent's signature for a secondary medication.
A review of five IHCPs indicated the following: 1) Four did not identify which staff were designated and trained to administer a prescribed medication. 2) One did not have the child's name listed.
The staff schedule was posted in the Admin office and not in an area that's easily visible to educators, parents, and visitors.
The following was observed: 1) In the HALLWAY outside the children's bathrooms, there was multiple spots of chipped paint on the red wall. 2) In the LEFT CHILDREN'S BATHROOM, the metal floor drain cover had a broken section, which could be a possible entrapment hazard.
