During the visit the written evacuation procedures were missing at two doors in the bee room. Also missing were maps at doors without direct access to outdoors.

Fall zones adequate depth of impact absorbing material
In the Monkey Room children who have asthma were not included on the allergy and medication list.

Mulch on the toddler playground was not adequate at the time of the visit and measured at 7 inches (9 inches is the required amount). The slides on the toddler playground did not meet requirements that they should be no more than 6 inches above the protective surface, at the time of the visit they measured at 11 inches for both the blue and green slides.

At the time of the visit there were a few areas of safety and cleanliness identified: in the duck room there was clutter next to the bathroom sink, clutter in a few areas of the room, cords that were loose near the phone and accessible to the children, a shelf that was loose and had a large lamp on it and cords hanging down that children had access to. In the CPPI classroom there was clutter in a couple areas of the room, molding separating from the wall and cords that were accessible to the children. In the dragon room there were areas of clutter. In the gym there was a trampoline. The regulation on trampolines is: 7.07 (12) Trampolines. Except for therapeutic equipment, the use of trampolines by child care children, whether indoors or outdoors, is prohibited.
At the time of the visit there was medication that was expired (epi pen and Benadryl). Medication was appropriately stored and labeled, medication administration was not observed during the visit.
