Fire inspection was expired at the time of the visit.
There were materials in the classroom that are torn and need replacing. During the visit the cubbies, shelving and a TV were not secured and tippable.

Staff records checklist was not complete at the time of the visit. Please review the items required on the list and ensure they are in the files of each educator. Missing were completion of essential trainings for a new educator, school age work experience forms for some educators, bi-monthly observations and yearly observations.

While reviewing files it was learned that there was a child who had asthma listed on their paperwork however there was no inhaler or knowledge of the condition.

Some of the files were incomplete, missing proof of BRC, first aid/CPR and school age work verification forms.

During the visit there was no thermometer in the fridge.

In room 122 broken door on built ins needs to be repaired, broken chairs to be removed and removal of other items that were leaning against the wall. The sink in room 209 needs to be cleaned and drained so the children can use it for handwashing.

At the time of the visit there was no staff schedule posted.
During the visit there was a short window of time that an educator was in the room with 21 children while another educator took a group of children to the bathroom and another educator went to check on those children and brought more to the bathroom.
During the visit educators were not always engaged with children, when the group broke into two groups there were times that the two educators were sitting in the corner of the gym while a group of children were in the gym without doing anything in the opposite corner of the educators.
During the visit the walls had chipping paint, floors had bumps and were lifting. Children's belongings were placed on floor and created clutter and tripping hazards.
The fire inspection was expired at the time of the visit.
At the time of the visit there were 32 children in a room that only has the the square footage to have 22 children.
Children and staff did not wash hands before snack.

During the visit the following was observed as issues with the indoor space: 1. The chairs in the infant/toddler room were dirty. 2. The floor in the toddler room appeared to be dirty. 3. In the school age room there was significant peeling paint. 4. In the infant toddler room the changing table pad was torn. 5. There was a mat in the preschool bathroom that is used for diapering that was torn.

During the virtual visit medication was stored appropriately in the first aid bags in two of the classrooms, the medication was labeled and in its original box, however one of the medications was expired.
