The child's Individual Health Care Plan included the child's medication dosage in MG (solid). However, the medication provided to the program is in liquid form (ML). The conversion dosage from MG to ML is unknown at this time.

The evacuation drill log from this past school year was reviewed. The September drill did not include the number of children evacuated.

1) The emergency numbers posted at the phone in the SA dedicated classroom did not include the address of the Health Care Consultant. 2) The emergency numbers posted at the phone in the cafeteria did not include the name, address and phone number of the Health Care Consultant.
1) In the girl's bathroom, the back portion of the wooden cover on the top of the radiator had separated and posed a potential hazard for children. 2) In the boy's bathroom, there appeared to be a leak coming from the bottom of the sink and there was water on the floor.

There were 2 groups of children in the cafeteria. Attendance included that there were 9 children present; however, there were 12 children present. The program must have a method of knowing who is present during all hours of operation.
In the cafeteria, there was food that was not kept in a covered trash container.

1) One child's Individual Health Care plan did not include all required information with doctor signature/date. 2) One child's IHC Plan included an emergency medication. The parent has not provided this emergency medication to the program with required documents.
