1) In the girl's bathroom, the back portion of the wooden cover on the top of the radiator had separated and posed a potential hazard for children. 2) In the boy's bathroom, there appeared to be a leak coming from the bottom of the sink and there was water on the floor.

There were 2 groups of children in the cafeteria. Attendance included that there were 9 children present; however, there were 12 children present. The program must have a method of knowing who is present during all hours of operation.
In the cafeteria, there was food that was not kept in a covered trash container.
