Current consent for administered medications
A sampling of children's files was reviewed. 1) Three children's enrollment forms packet was over one year old and has therefore expired. 2) One child did not have a current physical exam, lead test results, and immunizations.
A sampling of staff files was reviewed. 1) One staff file was missing a current physical exam. 2) Two staff who have been at the program for 6+ months did not have current first aid certification.
There was a fan in Preschool 2 and a bathroom that was accessible to the children.
In Preschool 2 classroom, the door in the back of the classroom did not have a written evacuation plan posted next to it.

1) The review of staff meeting documentation did not consistently include the start and end time of the meetings so that it could be determined if the program is meeting for a minimum of 2 hours monthly. 2) There was no written staff meeting documentation for June 2024.
1) In the Preschool bathroom, there was an area of pipe wrap where the inside of the pipe wrap was exposed. 2) In the Toddler classroom, there was some furniture which was partially obstructing the evacuation route from this rear classroom exit.
In both playgrounds, the depth of impact absorbing material under and around the structure and swings 6' fall zones did not measure a minimum of 9 inches.
The program has not been recording food served for AM snack/breakfast and PM snack on file for one year.

One child had 2 medications. The child's file did not include a completed Medication Consent Form for each medication.
1) One staff file reviewed did not include current First Aid certification. 2) Two staff files reviewed did not include a current annual evaluation.
On the day of the visit, the attendance reviewed in the Toddler 1 classroom included that for one hour, the classroom included 10 toddler children and exceeded minimum ratio of 9 children. The PD reported that a child who will be starting at the program came for a short visit without the parent present.
The thermometer in the Toddler 1 refrigerator read 50F. Food requiring refrigeration must be stored between 32F-40F.
In both outdoor playgrounds, there was insufficient ground cover depth in the use zones around the playground structures.
One child's allergy information was not posted in a manner that protects the privacy of the child.
The practice Evacuation Drill Log was reviewed. The last entry recording a monthly drill was 9/21. The program did not have evidence of conducting practice evacuation drills monthly as required.
One child's Individual Health Care Plan (IHC) was over a year old.
1) There was no evidence that all staff met for a minimum of 2 hours 6/21 - 9/21. 2) Staff meeting documentation did not include the start and end times or length of the meetings; therefore, EEC was unable to determine if all staff were meeting a minimum of 2 hours per month.
The program did not follow all Background Record Check policies and procedures.
Three children's files were reviewed. One child's file did not include a Transportation Plan.
In the Toddler 2 classroom, there was some pipe wrap where the covering was compromised.
The impact absorbing material at the end of one of the slide was not adequate depth.
The program did not practice an evacuation drill in 7/21.
One child's emergency medication dispenser did not include the exact amount of the dosage to be administered to the child.

Two children's medications and paperwork woere reviewed. 1) One child's Iindivudal Health Care Plan did not include possible side effects, potential consequences if treatment is not administred and the form was not signed/dated by the child's doctor. 2) One child's Authorization for Medication form did not include the reason for the medication and possible side effects. 3) One child's Indivual Health Care Plan was not signed/dated by the child's doctor.
At the visit, one staff and 2 children went outside from a classroom. One staff stayed in the classroom to finish feeding them their bottle. The attendance was inside the classroo,. The group that went outside did not have a method of knowing which children they had while outside.
In the Preschool 2 classroom, written evacuation plans and escape routes were not posted as each exit.
