The records were reviewed for seven enrolled children. One child needed an annual update form. One child was missing proof of physical and immunizations. The General Permission section of the enrollment packets did not reflect the locations that the educator takes the children on a regular basis; some were blank and some were generic.

There was no proof current progress reports for three of the enrolled children.
The required emergency information was not posted.

Progress Reports
There was access to portable fans in the nap room. There were diapering wipes, diaper bags, and a vacuum cleaner accessible in the nap room. Gate installed to prevent access to second floor stairway does not lock. The gate was down allowing access to clutter underneath the stairs.
The progress reports were reviewed for all 7 enrolled children. Six children needed proof of current progress reports.

Space (indoor / outdoor hazards) - R Apr 2025
The door to the nap room was closed with a child sleeping in the room. Discussed the door must be kept ajar at all times when children are sleeping in the nap room.
The Educator changed the child’s diaper on the floor. Discussed there must be two surfaces used for diaper changing. One surface must be able to be disinfected and the second surface needs to something that is disposable, is individual to each child, or can also be disinfected.

The thermometer in the refrigerator was not working. Discussed the temperature must be between 32 and 40 degrees. Please send proof of correct refrigerator temperature.
First Aid Kit

The first aid kit was missing tweezers.
There were cleaning products on the stairs, which is the access to the second exit. The stairs were gated, so the cleaning products were not immediately accessible to the children. Discussed making sure cleaning products are stored completely out of reach of the children in all licensed space.

The Educator's CPR and First Aid are both expired. Please send proof of current CPR and First Aid certification.
The First Aid kit was missing emergency cards for some of the enrolled children.
One (REDACTED) child did not have an enrollment packet. One child had an incomplete General Permission section. One child was missing the signature for the Acknowledgement of the Parent Handbook, had an incomplete General Permission, incomplete Medical Permission section, an incomplete Emergency Card. Three children were missing proof of current medical documentation.

The Educator did not have proof of current progress reports for two of the children. Three last progress reports were completed in February 2019. One of the children was recently enrolled and did not need a progress report. Two of the children were enrolled in (REDACTED), and were due for progress reports this month.
