The rubber matting under and around the preschool strucuture had a large amount of sand from the sand box. The sand made the rubber matting slippery and a potential hazard for children,

Medication
A sampling of children's files were reviewed. One child had 2 emergency medications. Both medication Medication Consent Forms were missing information regarding the medication's side effects.
The preschool playground was inspected. 1) The cover of the sandbox was hanging over the fence to the ground. This was a potential hazard to children. 2) Several roots were observed above the ground and a potential tripping hazard. 3) The rubber matting ground cover under and around the playground structure had sand throughout the surface and this made the rubber matting slippery and a potential hazard to children.
One child's emergency medication prescription label had expired on 5/7/24.

1) The program did not conduct a monthly fire drill in June 2023. 2) 2023 written fire drill documentation did not include the routes used to evacuate. The program needs to practice alternate routes as well as primary.

Refrigerator temp. between 32-40 degrees; working thermometer in refrigerator
1) Review of one child's file included that the Authorization for Medication form for a non-prescription medication was not singed by the child's doctor. 2) One child's file did not include an Authorization for Medication Form for a child who has medication.
1) One child with a chronic condition did not did not have an Individual Health Care Plan (IHC). 2) One child's IHC Plan did not include the consequences if the medication is not administered.
The K-Prep classroom refrigerator did not include a thermometer; therefore, EEC was unable to determine if food requiring refrigeration was being stored between 32F-40f.

Three children's files were reviewed. 1) One child's Individual Health Care Plan did not include consequences if the medication is not administered, side effects of the medication, staff who were trained on the administration of the medication and who trained staff to administer the medication. 1) One child's Medication Consent Form was signed by the parent on 1/3/2020 and is now over one year old and expired. 2) One child has 2 medications. Neither medication had a complete Medication Consent form completed by the chlid's parent.
