The licensor observed the following in the outdoor space: The mulch in the playground area with the play structure was lacking the proper 9-inch depth of mulch The 2nd playground behind the building has stairs with no barrier to prevent the children from entering the parking lot.

Program lacks sound administration. Based on the licensed capacity the program requires a full-time non-teaching Director. However the Director is assigned to a classroom for more than half of the day.
Child went to the bathroom without direct supervision.
Children were on their mats for over an hour with no quiet activities provided except books
No evidence of evacuation drills since 6/16/2021.
Educator covering the larger ‘4s’ classroom did not readily know where the emergency medications were located for the children in that classroom.
Three medications on site had either missing or invalid medication consent forms.

Impact material was not of sufficient depth for fall zone around tall climbing structure next to the pirate ship; Impact material was sparse around the pirate ship; sandbox did not have a cover and a good amount of sand was missing from the sandbox.
