Of the 3 staff files randomly reviewed the following was noted: 1. 1 file was missing the documentation of interview date, verbal verification of 1 reference, Yearly staff evaluation. 2. 1 file was missing documentation of interview date, verification of 2 verbal references, date of hire, evidence of a physical exam, evidence of a program orientation.

When observing outside the Licensor noted the following: 1. The Little Tykes plastic cube climber was within the 6-foot use/fall zone of the Geodome. 2. There was chipping/peeling paint on the shed. 3. The cover to one toy storage unit was broken and in need of replacement. 4. The tires on the pretend play car were disintegrated and full of holes. 5. The fire truck back window is a potential entrapment hazard.
Children did not wash their hands with soap and water prior to eating lunch, the Educator gave them hand sanitizer.

When reviewing IHCP and medications the following was noted; 1. 1 child’s prescription label for an [REDACTED] stated to discard on 5/16/19 and the actual [REDACTED] expired 9/18.
At the time fo the visit 2 children’s IHCP stated to administer [REDACTED] as an emergency medication however, [REDACTED] was not listed on the allergy/emergency medication posting as required.

At the time of the visit the following was noted: 1. 1 Educator's file did not have evidence of a completed Background Record Check (BRC)inside their staff file. 2. When the Licensor looked in the old BRC Manager and the new BRC Navigator there was no evidence that a BRC was ever run on this Educator under this program; therefore the Educator was hired prior to the program receiving a "Provisional" letter or "Suitable" letter from EEC stating the applicant could be hired.
At the time of the visit children’s IHCP’s listed [REDACTED] as an emergency medication however the medication was stored in the office and not with the child. This is not considered immediately available as required. The medication must be stored with the child’s [REDACTED] and must follow the child wherever they go.
At the time of the visit children were observed washing their hands for snack then being told to sit on the floor in the hallway to wait for their friends. Children were observed touching the floor with their hands, this is not considered proper infection control.
