In the "pre-K" room not all equipment was secured as required by regulation. The "cover" at the end of the heating unit was observed to be falling off of the unit in "pre-K" creating a potential hazard. The carpet had a seam down the middle and a section of carpet was observed to be pulling off the wall, creating a potential hazard. The mudroom carpet (outside of Pre-K leading to the outside) was observed to be curling up, creating a tripping hazard. Christmas lights were observed to be strung around the window in "toddler 2" but were not secured, the "wheel" that you wrap the lights around was observed to be resting on the window ledge, creating a risk to children. In the downstairs hallway, the key to the storage closet was observed to have been left in the keyhole, allowing potential access to this space. The doorframe to the gym space was observed to have been broken, leaving a jagged edge exposed to children, creating a potential risk.

It was observed in the "Toddler 2" classroom that children were divided into 2 groups, each on one side of the classroom. Children were told that they would "switch" sides- at the announced time which would, allow them to access the toys on the other side of the room. Children were observed to be redirected back to their assigned area of the classroom, even when they attempted to go over to the other side of the room. Children were told "no" they needed to stay on their assigned side until they were told they could move. This is not appropriate classroom management.

On 8/30/2021, the program was cited for the following: The "white shed" on the playground, was observed to be chipping white paint, accessible to both the infants and the preschool children, creating a potential hazard. On 2/22/2022, it was observed that this citation had not been resolved. The toddler swings were observed to have chains that are rusty, creating a potential hazard. There was a plastic climber observed on the toddler playground to be placed in the sandbox, without proper fall zones or the required impact absorbing material.

Child A's IHCP was reviewed at the time of the visit and the following was learned: the form was not signed or dated by their parent; the form was missing EEC required information, including who trained the educators, who was trained in the child's health care condition and the potential consequences of not administrating the medication.
