At the time of the visit staff was observed wiping a child's nose twice and did not wash her hands with soap and water. s
A review of IHCP's found some were not up to date Some IHCP did not have the parent signature on them.
IHCP and medication consent forms were not up to date for all children with ongoing medical concerns.
At the time of the visit the main attendance as well as the bright wheel attendance were not accurate.

During a review of the outdoor space, the following was learned: The chains and “S” hooks on the swing set were observed to be rusty and posing a potential risk to children. The “Railroad ties” that surround the sandbox, were observed to be rusty and pulling up in several spots, creating a hazard to children.
4 children’s records were selected randomly for review and the following was learned: [REDACTED]did not have evidence of current face sheet. [REDACTED] did not have evidence of a recorded date of admission. [REDACTED] did not have evidence of a current first aid and emergency medical consent form. [REDACTED]did not have documented evidence of a current off site consent form. [REDACTED] did not have evidence of a current transportation plan. [REDACTED] did not have documented evidence of a current progress report. [REDACTED]did not have evidence of a current developmental history form as required.

On the day of the visit, it was observed that Educator A left the classroom to open the front door for Parent A, leaving 7 children in the classroom.
In the upstairs children’s bathroom, the tile floor was observed to be pulling away from the hallway floor, creating a large gap that is considered to be a tripping hazard.
It was observed that an Educator assisted Child A in wiping their nose but neither the child or the educator were observed to wash their hands afterward.
