During the health and safety inspection, the following were observed: 1) Chipping paint was found near the trash bin in the Teddy Classroom. 2) In the Bunny Classroom, multiple ceiling titles were found stained.

During a review of the program's outdoor licensed space, the following was observed: 1) nails were coming up from the ground from a rotten wooden barrier on the back side of the playground, creating a potential hazard 2) In the back side of the playground, the area has been "sectioned off" for play due to some rotting trees overhanging the fence, creating a potential hazard to children.
During a review of cleaning and sanitizing, it was observed that the educator in the "bunny" room used only one step when washing the tables from snack, rather than the regulation required wash then sanitize.
At the time of the visit, the following was observed while reviewing all indoor licensed spaces: Chipping paint was found in the Bunny Preschool Classroom.

At the time of the visit, the following was observed while reviewing all indoor licensed spaces: 1) In the Teddy Preschool Classroom, a shelf was not properly secured, and a screw was exposed. 2) Multiple stained ceiling tiles were found in the Bunny Preschool Classroom.
Two records were randomly selected for review, and the following was learned: 1) One Staff did not have evidence of a current physical as required by regulation. 2) Two staff did not have evidence of a Staff Evaluation as required by regulation.

Supervision of Children
During the Bunny Preschool Classroom's transition inside from outdoors, Educators did not position themselves to see all children in their care. Educators were unaware that a preschool child was able to make their way into the bathroom to wash their hands.

During the visit, it was learned that the program is not conducting practice evacuation drills at least monthly. The Dircetor stated that she is in the process of creating a plan to ensure evacation drills are schedule on a consistant monthly basis.
Equipment and supplies: free of entrapment hazards
During the time of the visit, there wasn't evidence of a projected one-year operating budget that estimates income and expenses.
During a review of staff records, a file did not have evidence of a completed application and documentation of references.

Health Care Consultant Agreement
During the inspection of the outdoor space, a white wood beam with a birdhouse was not safely secured causing a potential safety hazard to the children.
The program did not have a current contract with a Health Care Consultant agreement.

This monitoring visit inspection recorded no violations or advisories.
During a review of a IHCP for a child with [REDACTED] medical condition, the following was noted: 1. The IHCP indicated the child is [REDACTED] medication however the medication was not onsite. 2. The IHCP did not have a parent’s signature. 3. There wasn’t evidence of a completed medication consent form for a medication outlined in the IHCP.

During the time of the visit the following was noted: 1. The siding of the building was cracked and punctured. 2. Chipping Paint was observed on the door entering the program and outdoor shed. The [REDACTED] is working with maintenance to correct the outdoor issues.

During the time of the visit, the fencing to the outdoor play area was weathered, splintering and had chipping paint. There was an unsteady rusty grate in the ground. Rusty nails were observed on the ground causing a potential hazard. There was a broke tool bench play set accessible to children. The [REDACTED] is working with maintenance to correct the outdoor issues.
