At the time of the visit the infant one classroom did not have accurate attendance. Attendance had 5 children signed in but there were 7 infants in the room.

During the visit an infant held toy bells while the educator was changing their diaper. The Educator took the bells and placed them on the floor without setting them apart for cleaning.
During the visit an Educator tried to have a toddler lie on her cot, but the child kept getting up. The Educator continued to physically lay the child down instead of offering a quiet activity. Per EEC regulations, when children choose not to sleep or wake up early, they must be offered quiet activities for the rest of the sleep or quiet activity period.
Educators in the Toddler classroom were not sitting with children during lunch time; instead, they were setting up cots.
At the time of the visit not all emergency medication listed on the children's IHCP was with the child at the program in the classroom.
Two toddler classrooms were out of ratio during rest time. One educator to seven children. in both classrooms.
One Infant classroom had two children present that were assigned to another classroom. At the time of the visit, it was learned that routinely classrooms are combined when numbers are low in a classroom.
