Random review of five staff files found the following: 3 had no evidence of a current physical, 2 did not have evidence of [REDACTED] and 1 file didn't have a staff information form.
A bathroom being currently used for toilet training had no toilet paper.
Diaper trash did not have an adequate cover.
An educator was observed sitting on a table in a toddler classroom. The educator was not modeling appropriate behavior.
Snack items were found open and not stored properly.
During health and safety inspection the following was found: dirty refrigerators, educators' personal drinks within reach of children and some dirty area rugs.
There is not adequate shade on one of the playgrounds.
Educators from one classroom left emergency medication in the classroom while the child was on the playground.
A child's [REDACTED] and [REDACTED] were in a Ziploc bag. The Benadryl had spilled out and all the contents of the bag were saturated.
It was learned an educator uses only a one-step process on tables and diaper changing tables and an educator was observed not washing hands immediately after changing a diaper.

A toddler classroom had no cleaner or disinfectant in the diaper changing area and the teacher couldn't tell Licensor where it was.
A large can of moldy applesauce was found in a classroom refrigerator.
Inspection of indoor space found the following: dirty area rugs, diaper changing tables not clean under and around diaper changing mats, dust on vents and lighting throughout the building, bathroom floors, refrigerators and tops of trash cans dirty and diaper disposal cover missing.
Review of 4 staff files found 1 file without evidence of a current physical and 1 file did not have completed evidence of teacher qualifications.
A preschool classroom had limited materials accessible for the children to be actively engaged.
A classroom had cough medicine for a child that had no medication consent on file signed by the child's doctor.
It could not be determined if evacuation drills are conducted due to the log could not be located.
Staff files are not being maintained with all required documents and accurate information. Review of 5 files found the following: 3 did not contain evidence of a current physical or evidence of immunity to MMR, 3 did not have PQ registration, 2 had no application, 1 had no references, 1 educator who works in infant classroom did not have Essentials training (which requires at least the safe sleep trainings to be in an infant room), 3 did not have evidence of program orientation.

It could not be determined if 2-hour staff meetings are held every month. Documentation did not include the year or timeframes and not all months were in binder.
A toddler classroom had emergency bag containing medication stored on the ground, accessible to children, in the playground.
When educators in one classroom were asked what they used to clean/disinfect they had to pull out the diaper trashcan from the cabinet to find the disinfectant in the far back of the cabinet. Cleaning and disinfecting bottles were not found accessible in many classrooms.
IHCPs for 2 children stated [REDACTED]and educators in classroom stated if the program had it, it would be in front office. [REDACTED] is part of the emergency plans and needs to be kept with other emergency medication and needs to be kept in the room/playground the child is at all times.
Review of 5 children's files found the following: 1 child's transportation plan was not signed/dated, 1 child's developmental history not signed/dated and 3 files did not have a current physical, IZ or lead screening.
An infant was observed sleeping in a crib in front of a window with the sun shining directly on them.
Not all bottles/food items were found labeled for the child they were intended for.
Not all educators in classrooms were aware if there were children with chronic medical conditions requiring emergency medication.
During health and safety inspection of all classrooms the following was found: half walls between classrooms were being used as storage for items such as educator's purses, personal drinks, diaper bags, attendance clipboards, etc... all of which were accessible to the children and pose potential hazards. Some refrigerators and diaper changing tables/counters (under and around changing pad) were found dirty, broken piece from front of sink counter in PreK, educators drinks within reach of children in multiple classrooms and on playground.
A wooden panel attached to brick building has rusty screws sticking out from it.
It could not be determined while reviewing 3 of 4 staff files (4 of the 5 staff files reviewed were said to be teacher qualified). There was not adequate documentation of work experience (ie: missing dates, one letter had no job title or age group, etc...) One file had documentation of the Cares Course completed in 7/2022, no work experience form and no further evidence of completion of Child Growth and Development.

An educator was observed putting a child on the floor after changing and telling them to go play without having them wash their hands.

The DPS 1 Classroom did not have a sufficient amount/variety of materials or toys accessible to children.
In all infant classrooms, multiple crib sheets were not found to be tightly fitted.
An educator was observed leaving their classroom out of ratio to get materials needed from another classroom.
Multiple classrooms had snacks that were not stored properly.
Multiple classrooms had bags of food in their refrigerators that were not labeled with an expiration date. Box of muffin mix was found opened and expired in main kitchen.
Emergency Medication found in the kindergarten room was not brought to the playground when the classroom transitioned outside.
During health and safety inspection the following was found: holes in floors that need to be filled and toilet seats were peeling and need to be replaced.

One refrigerator was found defrosting and dripping water inside and it could not be determined if the refrigerator in the kitchen was being maintained between 32-40 degrees.
Two educators providing coverage in a classroom were unaware if children in the classroom had emergency medication.
Not all snack items in classroom cabinets and in the kitchen were found sealed properly.
Not all children's medications were accompanied by a parental medication authorization form.
During health and safety inspection the following was found: Educators leaving items (ie: coolers, attendance clipboards, etc...) on half walls that could fall off and cause potential injury, not all floors and rugs were found clean and staff's personal drinks found within reach of children and pose a potential hazard.
Allergy postings did not include all children with chronic medication conditions and emergency medications.
During the visit it was learned not all staff are using the 2 step cleaning and disinfecting process on tables where children eat and on diaper changing tables.
No IHCP on file for a child with a chronic medical condition.
A child's [REDACTED] was stored in a classroom cabinet and not in the emergency bag and would not have gone outside to the playground and been available in an emergency because educators were unaware it was there.
