During the safety inspection of the program, the following was found:1) Chipping and peeling paint were found in the Young Toddlers Classroom. 2) A rug in the Young Toddlers Classroom was observed to cause a tripping hazard when a child walked over the corner of the rug and tripped.

In the Young Toddler Classroom, One Educator did not wash their hands with liquid soap and running water, using friction, and then dry their hands with individual or disposable towels after diapering multiple children.
During the visit, the following was noted: The toilet plunger and toilet brush were accessible to children. Adult scissors were on the table and not secured away from children. Not all cords in the downstairs young preschool classroom were safely secured.

It was observed in the Infant and Older Toddler Classroom that the educators did not keep one hand on the child at all times while completing a diaper change.

The following was noted: 1. Cords were unsecured, creating a potential hazard to children. 2. Children's bathrooms were cluttered with storage items 3. The children had access to a plunger and a can of Lysol in the preschool bathroom. 4. There was a mop and mop bucket accessible to children. 5. In the Preschool room chairs were stacked high, creating a potential hazard to children.
There was a sensory board that was weathered with different metal pieces rusting and broken.
1. Training toilet seats were observed on the floor in the children's bathrooms. 2. The rest mats were observed to have torn edges.
This monitoring visit inspection recorded no violations or advisories.
During the time of the visit, the following was noted: 1. Children did not wash their hands after toileting. 2. Staff did not wash thier hands after assisting children with toileting.

Through a review of IHCP's the following was noted: 1. Two IHCP's were incomplete. 2. One medication form did not have a reason for [REDACTED] medication.
Written evacuation procedures were not posted next to each emergency exit as required.
At the time of the visit, the following was observed while reviewing all indoor licensed spaces: 1) Multiple shelves were not secured in the Older Toddler and 5's Classrooms. 2) Peeling/chipping paint was found in the Older Toddler and 5's Classroom. 3) Multiple screws were exposed on the wall in the 5's Classroom.
The following was found at the time of the visit: 1) Bug spray was found in an unlocked cabinet in the Older Toddler Classroom. 2) A toilet brush was found in children's reach in the 5's Classroom.
A staff person did not know if any children in the preschool classroom had emergency medications.
Through a review of staff records the following was noted: 1. Four files were missing documentation of interviews. 2. One file did not have verification of references.
During the visit, an educator was observed not wash [REDACTED] hands after assessing a child with wiping [REDACTED] nose.
Not all the children were directed to wash their hands before eating lunch.

During the time of the visit, children were observed using a toilet training seat that was not washed and disinfected after each use.
During the visit, an educator was observed serving children drinking water from the hand washing sink.
Not all lunch boxes, cups and food containers were labeled with the name of the child for whom they are intended.
A child's [REDACTED] medication was left in the classroom during outdoor play, therefore the medication was not readily available to the child.
Tables were not washed and disinfected before the children sat down for lunch.
