The educator was using a bassinet that did not have an adequate mattress and there was a plastic tray under the mattress which did not belong with the bassinet. The educator did not have documentation the bassinet met current safety requirements for infants. The educator added an additional mattress to another port a crib where an infant rested which is not considered safe or appropriate. The educator set up two appropriate port a cribs prior to resting the infants at the time of the visit.
