All of the children's records were reviewed, one child did not have proof of lead screening, one child did not have proof of a physical and two children did not have an individual health care plan for asthma.
The educator did not have a medication consent form for two children with an (REDACTED). The (REDACTED) was not in its original container with original label containing the name of the child affixed.
Two gas cans were accessible outside of the kitchen exit.
In the bathroom, there were razors, lotions, hair products and personal hygiene items accessible on the sink.
The carbon monoxide on the first and second floor were tested and not functioning.
The smoke detector on the second floor and bottom of the stairs on the first floor was tested and not functioning.
