Upon a review of ten Children's Records, the following was identified: -Two individual children did not have a current and complete Medication Consent Form for a prescribed medication.
A walk through of the facility indicated the following: -On the stage area, there were (10) stacked tables, (7) stacked chairs, and a large television that was not secured and tippable that were all accessible to the children.
Upon a review of thirteen (13) medications stored onsite, the following was identified: -One medication was not stored in its originally prescribed container with affixed prescription label.
Upon a review of ten Children's Records at the program with Individual Health Care Plans, the following was identified: -Six Children's Records did not have evidence of a current and complete IHCP. -Two Children's Records did not have evidence of an Individual Health Care Plan for two individual children diagnosed with (REDACTED)

Upon a review of the BRC Navigator system, the following was identified: -One educator has a "pending" determination and fingerprints were processed in 2/2020. This educator's BRC needs to be renewed. -One educator listed as a substitute is listed as "pending." A fingerprint letter was issued to the educator on 1/24/23. -One educator is deemed suitable but is affiliated with the Supervised Remote program. This educator's BRC needs a suitability transfer to the correct program.
Upon a review of medication stored at the program, one (REDACTED) but the medication was in a box with a prescription label affixed that indicated the medication expires (REDACTED)
This capacity increase visit inspection recorded no violations or advisories.
Upon a review of three Children's Records at the program: -Two did not have current and complete Medication Consent Forms. This means in the event of a potential emergency, the program did not have parental permission to administer life saving medication to two children in the program.

The Staff Schedule posted listed the same educators at two site locations: Coombs and Quashnet. Each site must list consistent staffing assigned to consistent groups of children.
Upon a review of three Children's records at the program: -Two did not have complete Individual Health Care Plans. Please ensure you are indicating the name of the educators trained in the child's medical condition and who trained those educators on the IHCP Form.
This investigation inspection recorded no violations or advisories.
During the visit, an allergy list was posted but it did not protect the privacy of each child.
